POST /patients/{patient_id}/documents

multipart/form-data upload.

Parameters (form fields):

• file (required)
• title (string, optional)
• type (string, optional — one of the document-type keys; defaults to other)

An unrecognized type returns 422 DOCUMENT_TYPE_INVALID.
